Registration Certificate

You've signed the forms and paid your fee. This is an example of what an AKC Registration Certificate will contain.

Name of your dog	This is your dog's formal name like Blue Beezwax Champagne. It must be long enough or unusual enough so that it doesn't already exist in the Registry. Over time, it becomes increasingly difficult to not duplicate names, hence the long and strange concoctions.
Breed	One of the breeds recognized by that Registry (150 for the United States' American Kennel Club or AKC)
Color	Basic Description like Golden
Sire	The father's name, registration number, date of the stud book he is registered in, OFA or other Registry information where appropriate.
Dam	Same as above for the mother
Breeder	Name of the breeder
Owner	Your name and current address
Number	The unique registration number assigned by the Registry to your dog
Sex	Of your dog
Date of birth	Of your dog
Date of Certification	The date this certificate was issued by the Registry.

On the back: a blank form that can be sent to the Registry to record a future transfer of ownership (if any) of your dog.
